You give all guests a piece of paper {writing or construction} and have the mother to be stand up so everyone can get a look at her profile. Have them take the paper, fold it in half, put it behind their backs, and, with their hands, only tear out what they perceive as her form. When everyone is done you have them open them up and the best one is picked by the mother to be. They get a gift! If there is more than one that she likes that person also gets one.

Then put the name of the creator on them and put them in the baby's book. Lots of fun.
